Abstract
Pressure ulcers are major problem in health care resulting in significantly higher healthcare costs, healthcare utilization, prolonged hospital stays, pain and discomfort while also negatively impacting a patient’s morbidity and mortality. Critically ill patients are at high risk for development of pressure ulcers with incidence ranging from eight to 40 percent.A single severe pressure ulcer can cost $50,00.Confusion exists about which of the multiple prevention strategies should be adopted. The purpose of this paper was a review of recent evidence based practice to determine the most effective strategies for prevention of pressure ulcer in the adult ICU. Clinical practice guidelines, systematic reviews, RCT’s and quasi-experimental studies were reviewed. There was good consensus on five prevention strategies(turning, skin assessment, skin care, superior support surfaces and staff education) and strong support for the formation of a multi-disciplinary prevention team. All sources recommended conducting multiple prevention strategies and the above five can be practiced as a care bundle –a familiar ICU strategy for preventing other adverse events.Studies also highlighted the importance of experts at the bedside, such as wound care nurses, coaching the ICU nurses and ensuring compliance with the care bundle. Instituting a pressure ulcer care bundle using the above EBP recommendations, with strong system and hospital leadership support,can be an effective strategy firstly to reduce patient harm, suffering, morbidity and mortality and secondarily to promote cost effective care in the adult ICU.
